Health Care Refferals
NURSING CARE
we offer referrals to licenced Registered Nurses and licenced Practical Nurses service agencies that can provide you with private hire or insurance paid services for nursing care that can provide a nurse for medication administration (Neupogen, Lovonox, Insulin, etc.), wound care, including Wound Vac IV therapy, such as antibiotics, IV fluids, TPN, chemo, etc. We will do all the paper work , and complete referral packet forms, such as BRIGGS, M11Q, 485s.
Home health Aides
We make referals to angencies that are certified or licenced to provide home health aides, companions, or private hire aides: Aides that can provide with personnel help in your home for shopping, cleaning, personnel care such as bathing, dressing, meals, etc.
SOCIAL WORKER
We make referrals to community base Social Workers who will visit you in your home for social issues, such as problems with your SSI, SSD, housing, health insurance, foodstamps, Medicaid, Social Security, Medicare, and Medicare-D medications. Our Services can be billed to some insurances or private pay.
THERAPIST
Referral to Physical and Occupational and Speech therapists: Services will be provided in the homes of the homebound.
VISITING DOCTORS
Referral to Visiting Doctors and NPs who will visit the homebound in their home for their medical needs. Service is especially provided to those who can not get out to regular MD appointments. Because of poor gait, inability to ambulate or walk well, and poor mental status. |
